How worried should I be about my girlfriend getting pregnant, even if I use a condom? I read that perfect use typically results in a 2% risk, but I'm the type to get easily worried, even with good odds in my favor. Besides that, I don't doubt that there are any number of things that could worsen those odds.
It's also worth mentioning that she isn't on birth control herself, as she doesn't want to mess with her hormones, deal with the daily pills, etc. since we aren't having sex on a very frequent basis.

1%-2% chance is about right. What you might want to do is try not having sex when she is ovulating. Ge an ovulation calendar or go see a doctor to find out when she drops her egg every month. There's only a three or four day gap where you can actually conceive a child, and that happens right about in teh middle of a menstrual cycle. Having sex right before or right after a period should not yield a baby, though using a condom is definitely something to do even then.
